Frequently Asked Questions

  • Signs & symptoms of pelvic floor dysfunction can include hip & low back pain, urinary leakage / urgency / frequency, constipation, pelvic pain, pain with intercourse, + more.

    Additionally, if you are hoping to become pregnant, currently pregnant, or have given birth (both Cesarian birth & vaginal birth) pelvic floor physical therapy is a must!

  • Centered Physical Therapy & Wellness is considered an out-of-network (OON) clinic, meaning all of our PT services would be covered under any OON coverage you may have. This allows us to have 1:1 appointments and be fully committed to patient centered care by having full autonomy over your individualized treatment. Additionally, we are able to cater to your needs rather than be dictated by insurance coverage.

    We will provide a detailed receipt upon request for you to submit to your insurance company, though we cannot guarantee reimbursement.

    You can use your health savings accounts (HSA) and flex savings accounts (FSA) to pay for treatment as well as credit card, check, and cash.

  • Wisconsin is a Direct Access state, this means patients can see a physical therapist without needing a healthcare provider's prescription.

    However, some insurance plans require a prescription to cover services. If you plan to submit your claim to your insurance company, check with your plan if you need a referral for reimbursement.

  • The first session includes a detailed conversation about your symptoms, how it impacts you personally, and your goals for the future.

    Following this, a movement screen and assessment will be completed including muscle strength & length testing to determine movement changes and underlying causes of your symptoms.

    Based on our conversation and what we find in the movement assessment, you will be provided with a home program to continue to address your symptoms outside of the clinic. This varies significantly person-to-person as everyone is different!

    If being seen for pelvic floor physical therapy, the assessment varies based on your symptoms. We may discuss an internal pelvic floor assessment to determine the muscle function of the pelvic floor directly! This is certainly not required though it can be helpful for a holistic approach to your care. This will be discussed with you during the session if it is indicated.

  • Your appointments and well-being are very important to us. We certainly understand that sometimes, unexpected delays can occur, requiring schedule adjustments. If you need to cancel or reschedule your appointment, we respectfully request at least 24 hours notice.

    Our Policy:

    Any cancellation or reschedule made less than 24 hours will result in a cancellation fee. If you are more than 15 minutes late for your appointment, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our very best to reschedule your service for another time that is convenient to you. We require a credit card to hold your appointment. Cancellation fees will be charged to your card on file. In the event of a true, unavoidable emergency, all or part of your cancellation fee may be applied to future services. We also ask that if you are sick with any symptoms or have been exposed to Covid-19 that you please contact us to delay your appointment as soon as possible.

  • All session are one-on-one with Dr. Sarah. Evaluations are 75 minutes and follow-up session are 50 minutes.

  • Definitely!

    It is worth noting, we currently don't perform internal assessments/treatments for male pelvic floor dysfunction, though there are certainly many overlaps between orthopedic involvement and pelvic floor dysfunction including pubic pain, pressure management, low back pain, etc.
